Residents of northern Alameda County have big reasons to smile!

The Foundation Board of Directors, along with Medical Center
Aministration and Medical Staff, engages in a careful process to
determine Foundation fundraising priorities by identifying the pressing
needs facing the community and the resources of the Medical Center.
Priorities have been selected for the next two fiscal years (2006/2007
and 2007/2008).

CANCER SERVICES

Comprehensive Cancer Services at the Medical Center are designed to provide screening and outreach to the community, navigation for all patients referred for breast evaluation, support groups for all cancer
patients, and a multi-disciplinary Tumor Board, which is a collaborative decision making process for treatment of patients diagnosed with cancer.
Some programs within this service are:

  • Breast Cancer Early Detection Program - provides free mammograms to women over 40, clinical breast exams and free treatment for women diagnosed with breast cancer.
  • The Breast and Cervical Cancer Control Program - provides free mammograms to women over 40, clinical breast exams and pap smears for low income women without health coverage.
  • Cancer Navigator Program - provides patients, their caregivers and families with practical support to help them overcome barriers to care. The Patient Navigator assists in mitigating some of the economic and social barriers to cancer treatment. | more >

COMPREHENSIVE CHRONIC DISEASE MANAGEMENT
INITIATIVE

Which includes:

  • Electronic Health Record System - The Electronic Health Record is designed to improve quality of care and patient safety, permitting improved access to important clinical information, direct entry of data by clinicians and other users, and clinical decision support tools at the point of care.
  • Diabetes Management Program – This program provides support for diabetics and their families by linking families with a healthcare team, working to reduce the number of harmful complications from diabetics, decrease the onset of disease in family members, and reduce the cost of care.
  • Childhood Obesity Program – The Childhood Obesity Prevention Program is part of the Diabetes Management Program and is a comprehensive approach to the issue of overweight and obesity. Targeting overweight and obese children, nutritionists and doctors work with families to assess eating habits and sedentary lifestyles.
  • Asthma Management Program - Asthma is one of the most common chronic diseases worldwide and the prevalence is increasing, especially among children. Asthma care requires a partnership between the patient and the medical staff. The aim is to provide patients with the ability to control their asthma with guidance from the health care professional. | more >

YOUTH VIOLENCE PREVENTION PROGRAM

The Youth Violence Prevention Program is a partnership between the Alameda County Medical Center Highland Campus and “Caught in the Crossfire” - an intervention program of Youth ALIVE! - dedicated to preventing youth violence.

This partnership works to prevent violent injury reoccurrence and retaliation among youth between the ages of 12 and 20 who are being treated at Highland Hospital for violent injuries. It is a visitation program in which young adults from the same communities and having had similar experiences as the victims are trained and hired to work intensively as models, mentors and case managers with the youth and their families for up to one year.

The program served over 700 youth since its inception in 1994 and has helped reduce the cycle of violence-retaliation-violence. It has been
able to provide services to over 500 youth since its inception in

MODEL NEIGHBORHOOD PROGRAM

The Model Neighborhood Program is a year-round youth development program designed to promote positive alternatives for our youth. and is comprised of four core components: Health Career Training Internships, Health Education, Graduate Board, and a Parent Participation Project. By creating a supportive and concerned environment, the Program equips our youth with practical tools to become healthy and productive citizens. | more >
